En revanche, personne n\u2019aime penser au risque d\u2019infarctus ou d\u2019arr\u00eat cardiaque qui plane au-dessus du parcours.<\/p>\n<p>Si, malheureusement, ces \u00e9v\u00e9nements catastrophiques se produisent, ils ne sont heureusement pas trop fr\u00e9quents.<\/p>\n<p>On d\u00e9nombre plus d\u2019infarctus, d\u2019arr\u00eats cardiaques et de d\u00e9c\u00e8s associ\u00e9s chez les hommes de plus de 40\u00a0ans et lors de marathons (bien que des femmes et des coureurs plus jeunes en sont aussi victimes, et que des distances plus courtes ne nous en mettent pas \u00e0 l\u2019abri). L\u2019ath\u00e9roscl\u00e9rose, c\u2019est-\u00e0-dire le durcissement des art\u00e8res, est habituellement \u00e0 l\u2019origine de ces probl\u00e8mes cardiaques.<\/p>\n<p>L\u2019aspirine a fait ses preuves depuis longtemps dans la pr\u00e9vention et le traitement de tels accidents aupr\u00e8s de patients qui pr\u00e9sentent des facteurs de risque d\u2019ath\u00e9roscl\u00e9rose.<\/p>\n<p>De nombreux m\u00e9decins de course se demandent aujourd\u2019hui si la prise d\u2019une aspirine le jour de la course pourrait r\u00e9duire le risque d\u2019accident cardiaque chez les coureurs.<\/p>\n<p>Bien qu\u2019aucune \u00e9tude n\u2019ait encore v\u00e9rifi\u00e9 cette hypoth\u00e8se, celle-ci est suffisamment d\u00e9fendue pour qu\u2019un groupe de m\u00e9decins de course (l\u2019Association internationale des directeurs m\u00e9dicaux de marathon) diffuse l\u2019avis suivant\u2009:<\/p>\n<p><strong>\u00ab\u2009\u00c0 moins d\u2019une contre-indication particuli\u00e8re, les coureurs de fond, surtout les hommes \u00e2g\u00e9s de plus de 40 ans, devraient prendre une aspirine avant une course, moyennant l\u2019approbation de leur m\u00e9decin, avec qui ils auront discut\u00e9 des risques et des avantages d\u2019une telle pratique.\u2009\u00bb<\/strong><\/p>\n<p>Ces m\u00e9decins proposent que tous les hommes de plus de 40 ans prennent une aspirine \u00e0 faible dose (81\u00a0mg) le jour d\u2019une course de 10\u00a0km ou plus, \u00e0 moins d\u2019une contre-indication.<\/p>\n<p>Quelles sont ces contre-indications?<\/p>\n<ul>\n<li>Une allergie \u00e0 l\u2019aspirine;<\/li>\n<li>Un trouble h\u00e9mostatique (l\u2019aspirine accro\u00eet tr\u00e8s l\u00e9g\u00e8rement le risque de saignement);<\/li>\n<li>La prise d\u2019anticoagulants (voir le point pr\u00e9c\u00e9dent);<\/li>\n<li>Des ant\u00e9c\u00e9dents d\u2019ulc\u00e8res \u00e0 l\u2019estomac ou de saignements gastro-intestinaux;<\/li>\n<li>En th\u00e9orie, l\u2019aspirine peut aggraver des probl\u00e8mes r\u00e9naux caus\u00e9s par l\u2019\u00e9puisement d\u00fb \u00e0 la chaleur et \u00e0 la d\u00e9shydratation.<\/li>\n<\/ul>\n<p>Cet avis a soulev\u00e9 une certaine controverse et donn\u00e9 lieu \u00e0 d\u2019int\u00e9ressantes discussions dans les forums sur la course et dans la litt\u00e9rature m\u00e9dicale.<\/p>\n<p>Que devriez-vous faire?<\/p>\n<p>Si vous prenez d\u00e9j\u00e0 de l\u2019aspirine en raison de facteurs cardiovasculaires, <strong>continuez <\/strong>de le faire.<\/p>\n<p>Si vous pr\u00e9sentez des facteurs de risque d\u2019ath\u00e9roscl\u00e9rose (hypertension, diab\u00e8te, tabagisme, etc.), <strong>songez s\u00e9rieusement <\/strong>\u00e0 prendre une aspirine \u00e0 faible dose (81\u00a0mg) avant de courir (parlez-en \u00e0 votre m\u00e9decin).<\/p>\n<p>Si vous \u00eates un homme \u00e2g\u00e9 de plus de 40 ans, que vous ne pr\u00e9sentez pas de facteur de risque notable et que vous courez sur des distances de 10\u00a0km ou plus, soupesez le risque d\u2019une telle pratique.
